POLICIES AND PROCEDURES

SELF PAY POLICY

Fees for office visits, medical aesthetic services and treatments, laser services, and medications ordered through the clinic are the responsibility of the patient and due at time of service/order.

We do NOT accept insurance for services provided in the clinic.

If you elect to use your health/prescription insurance for medications sent to your pharmacy, we will complete one prior authorization (PA), if requested. We do not correspond with the insurance company.

PAYMENT OPTIONS

We accept Care Credit, HSA/FSA cards, cash or checks, as well as all major credit cards.

REFUND POLICY

We kindly ask that payment is made at the time of service. Please note that all payments for the services provided are non-refundable. However, we truly value each of our patients and are committed to working with you to accommodate your needs. If you have any concerns or specific circumstances you’d like to discuss, please feel free to reach out to us. We’re here to help and ensure you have a positive experience!

SCHEDULING APPOINTMENTS

Please call our office at 337.415.0083 to schedule an appointment with Dr. Holley Kelley or our Laser Technician. We will do our very best to accommodate your needs within our clinic hours.

CANCELLATION POLICY

Each time a patient misses an appointment without proper notice, another patient is prevented from receiving care.

We kindly request that you give at least 24 hour notice if you need to cancel or reschedule your appointment. Voicemail and text messages are acceptable notifications when sent at least 24 hours prior to the scheduled appointment.

We reserve the right to collect payment, in advance, for appointments rescheduled due to a missed appointment (no show), or an appointment which, absent a compelling reason, was not cancelled with satisfactory notice (at least 24 hours).

Payment will be collected when rescheduled appointment is made and is non-refundable if not cancelled or rescheduled with satisfactory notice (at least 24 hours). Multiple missed appointments in any 12 month period may result in termination from our practice.

We appreciate your understanding and cooperation as we strive to best serve the needs of all of our patients.

DIGITAL INTAKE & CONSENTS POLICY

Electronic intake forms and consents can be more reliable and convenient than paper forms. We will send intake paperwork, consents, and other communications through the Patient Portal to securely collect this information prior to your appointment, and/or as needed. If you are unable to complete these forms prior to your visit, please arrive 10 -15 minutes ahead of your scheduled appointment time and we will be happy to assist you.

SHARING YOUR INFORMATION

SMS consent and phone numbers collected/captured for SMS purposes will not be shared, sold, or disclosed with any third parties, or any other individual for any type of marketing purposes, or affiliate marketing.
